Support Coordination

At NSDS, we understand how important it is to find the best options to meet your needs.  Our Support Coordinators help you to identify and find the supports and services needed to reach your goals.  Our Support Coordinators take the time and effort to understand what a meaningful life looks like for you, and where you want to go.  

 

We will support you to:

  • Understand your NDIS plan and goals

  • Understand what supports and services are funded by the NDIS

  • Provide you with information about what supports and services are available

  • Negotiate services and pricing to develop your budget that lasts for the life of your plan

  • Empower you to access and coordinate supports identified in your plan

  • Link you in with your local community

  • Develop your capacity to confidently manage your NDIS Plan over time

Daily Living Skills

 
Helping you to learn to live more independently with the right supports

At NSDS we believe you can learn new skills and develop existing skills with the right supports.  

We can help you to learn daily living skills such as:

  • Personal care (e.g. bathing, dressing, grooming)

  • Wash and iron clothes

  • Cook meals

  • Wash the dishes

  • Clean the house

  • Look after the garden

  • Go shopping 

  • Make appointments 

  • Money and Budgeting

  • Travel on public transport or in a taxi

 

By sharing the jobs around the house, and using step-by-step methods, we can help to give you more independence.

Daily Living Skills can be funded through:

  • Core Supports – Assistance with Daily Living; or

  • Capacity Building – Improved Daily Living Skills.

Home and Garden Maintenance

At NSDS we can take care of all your house and yard responsibilities such as:

  • Cleaning

  • Wash and iron clothes

  • De-cluttering

  • Gardening

Supported Independent Living

 

Helping you to find and live in a place to call home. 

At NSDS, we will work with you to find a place to call home.  We’ll get to know you and look to find housemates with similar interests who you will get along with.  We will also support you with day-to-day tasks around the house like cleaning and laundry, cooking, grocery shopping and personal care.

Supported Independent Living is funded through:

  • Core Supports – Assistance with Daily Life.  

It does not include the cost of day to day expenses, rent or transport.

Life Skills Development 

 

Helping you develop the skills you need to make the most out of life.

Any skill that is useful in your life can be considered a life skill.  Using a computer or catching public transport are, for most people, useful life skills.  However, the most important life skill is the ability and willingness to learn.  If you would like to learn a new skill, then our life skills program is for you!  At NSDS we provide training and development across a range of like skills, including:

  • Digital Technology – Learn to use software on computers, and Apps on phones and tablets.

  • Literacy and Numeracy –Develop or refresh your numeracy and reading skills with one of our qualified teachers.

  • Cook and Eat – Follow recipes and create meals to eat together.  

  • Transport and Travel – Learn how to plan a journey, MYKI system for catching buses and trains, and how to organise a taxi. We can also help you obtain your learner’s driving permit.  

  • Art – learn drawing and painting techniques in this fun and creative course.

  • Get Active – fun sporting activities for all ages, abilities and levels.

  • Music – learn a musical instrument at your own pace with the support of our specialist music instructors.

  • Life Skills – talk in a safe environment about social skills, regulation of emotions, sexuality, relationships and personal safety.

Community Connections

Helping you to take part in your community

 

At NSDS, we understand how important it is to be able to get out and about, foster friendships with people with similar interests, pursue hobbies, stimulate creativity, and try new things.  We can support you to participate in group activities or with a 1:1 arrangement. Everyone is different so the choice is yours!

Community Access

NSDS can provide individual support any day of the week to help you participate in community activities of your choice. We are able to support you with one off arrangements, or on a continuous basis for activities such as:

  • Going shopping, banking or to the post office

  • Visiting friends and family

  • Attending groups or activities

  • Attending appointments

  • Community events

Group Activities

Our group activities program is stacked with fun things to do, baking, arts and craft, sweet decorating, pool competitions, lego group, music and dancing, board games, sporting activities, pet therapy, personal pampering (manicures and makeups) and movie screenings.

Active Recreation

If you’re looking to be more active, then come and try one of our sports or active recreational activities.  We offer lawn bowls, dance classes, social tennis, fishing, swimming and more.  Active recreation activities vary according to the season so there is always something new to try!  

Out and About

At NSDS we understand the importance of getting out and about and socialising with like-minded people. 

Our weekly bus trips are a fun filled social activity.  It could be a trip to Geelong for fish n’ chips by the Bay, a scenic drive through Blackwood forest and a picnic amongst the daffodils, or a day trip to Daylesford and the lavender farm.  The destinations are always changing based on your feedback and interests.  

If you have an idea for a group outing, please let us know.  We would love to work with you to organise an outing which suits your interests. 

Movie Night 

If you like a good movie, then our fortnightly movie and games night is for you.  Come and share the laughs, fun and friendship!

Youth Headquarters (YHQ)

If you are aged between 18 – 25 years old, you can become a member of our Youth Headquarters (YHQ).  Hang out with other young people at our Hub!  As a member of YHQ, you can participate in pool competitions, book club, movie club, the gaming area, cooking and catering, first steps to barista training, and sports activities.
